About us

StickyJ Medical ID and sister company Thoughtful Impressions have been family owned and operated for more than 21 years. The company is led by CEO Lori Torman, who has an eye for great quality and classic styles. In the early days of medical id bracelets (we're talking 1960's), medical alert bracelets were pretty boring -- basically engraved metal tags with curb link chains. The same styles were offered for children, just smaller (no wonder kids didn't want to wear them). And, by the end of the 20th century things hadn't improved that much. Lori was among the first to recognize that medical id's could be fashion conscious, and in 2005 launched designs in leather as well as stainless, and introduced polyester kids medical id bracelets that had cute designs. The sport strap line of child medical id's she introduced nearly 20 years ago is still the most popular line of children's medical jewelry on the market today. And, we now produce more than 100 different styles of medical alert bracelets and necklaces for men and women. California

StickyJ Medical ID is an authorized provider of California Do Not Resuscitate and California POLST bracelets and pendants.

  • California requires that your DNR or POLST medallion or bracelet contain a specific message on the front depending on whether it's DNR, POLST, or POLST with Do Not Resuscitate. When you send us your DNR or POLST form, we will adjust the front engraving to conform with your document and state regulations.
  • The back of your POLST bracelet must have our 24-hour toll-free verification number, the patient's identification number (which we will provide), and the patient's full name. StickyJ Medical ID provides the required 24/7 telephone verification service at no additional charge.
  • You can either scan the document and upload it at the time you place the order, email it to CustomerService@stickyj.com, or fax it to 727-499-0989.  
  • QR Code Option for Quick POLST Access by EMTs

    For a one-time additional fee of $9.95, StickyJ Medical ID offers an optional QR Code feature with your California POLST bracelet. If you select this option, you can send us a copy of your DNR order, which we will securely digitize and upload to our protected online database. The QR Code which we'll engrave on the back of your bracelet, allows emergency responders to use their smartphone to scan and immediately view an image of your POLST or DNR order instead of having to phone our toll-free verification service.  This can be extremely useful when time is of the essence.

    To add the QR Code option, you must either scan-and-upload OR mail a copy of your Do Not Resuscitate order to StickyJ Medical ID. Once received, we will securely store your document online, making it accessible to EMS personnel whenever necessary.  (Note: if you are planning to scan and upload your POLST or DNR, we recommend using Adobe Scan.  It's a free app available for both Apple and Android phones).

Important Legal Notice

Please be aware that the bracelet itself is not legally sufficient for EMS personnel to withhold resuscitation efforts. In the event of a cardiac emergency, you must have a copy of your actual Do Not Resuscitate Order with you, and it must be accessible to emergency responders. Only a medical professional can issue a Do Not Resuscitate Order. Additionally, some state-approved agencies may offer their own DNR bracelet options.
