- The patient’s first & last name must be engraved on the first line of the back of the bracelet, however there is room on the back to add an emergency contact phone number, if desired.
- You can either scan the document and upload it at the time you place your order, send it to us by email to CustomerService@stickyj.com, or fax it to us at 727-499-0989.
