About us

StickyJ Medical ID and sister company Thoughtful Impressions have been family owned and operated for more than 21 years. The company is led by CEO Lori Torman, who has an eye for great quality and classic styles. In the early days of medical id bracelets (we're talking 1960's), medical alert bracelets were pretty boring -- basically engraved metal tags with curb link chains. The same styles were offered for children, just smaller (no wonder kids didn't want to wear them). And, by the end of the 20th century things hadn't improved that much. Lori was among the first to recognize that medical id's could be fashion conscious, and in 2005 launched designs in leather as well as stainless, and introduced polyester kids medical id bracelets that had cute designs. The sport strap line of child medical id's she introduced nearly 20 years ago is still the most popular line of children's medical jewelry on the market today. And, we now produce more than 100 different styles of medical alert bracelets and necklaces for men and women. StickyJ Medical ID Jewelry is the State of Wisconsin's authorized vendor for permanent-type DNR bracelets, available with standard lobster or safety clasp.  We also provide Wisconsin POST bracelets indicating whether your POST form is located in Purse or Wallet.  A DNR bracelet from StickyJ takes the place of having your Do Not Resuscitate order with you, but if you have a POST order from your physician you are required to carry a copy of the POST order with you at all times.  Our POST bracelet helps to inform emergency medical personnel that you have a POST order, and it helps them locate the order (in your purse or wallet).   Note: Because of our agreement with the state to offer the bracelets at a deeply discounted price, coupons are not accepted for the purchase of a Wisconsin DNR bracelet.   LEARN MORE HERE ABOUT WISCONSIN DNR BRACELETS FROM WI DHS..  

  • The patient’s first & last name must be engraved on the back of a DNR bracelet, however there is room on the back to add an emergency contact phone number, if desired.  POST bracelets don't have this requirement.
  • You can either scan the DNR or POST document and upload it at the time you place your order, send it to us by email to CustomerService@stickyj.com, or fax it to us at 727-499-0989.
  • For orders by mail, a copy of your completed Wisconsin Do Not Resuscitate form signed by your doctor (form F44763 ) or Wisconsin POST form must be mailed to StickyJ Medical ID along with your payment of $42.49 (or $47.44 if ordering a bracelet with safety clasp) and your completed WISCONSIN DNR BRACELET ORDER FORM prior to releasing your order. QR Code Bracelet Option for Quick DNR Access (optional)

For an additional $4.95, StickyJ Medical ID offers an optional QR Code feature with your medical ID bracelet. If you select this option, we will digitize and upload the form to our secure cloud database. The QR Code engraved on the back of your bracelet allows paramedics and emergency responders to use a smartphone to scan and immediately view an image of your DNR or POST order. While the DNR bracelet itself efficiently communicates your wishes, having the actual physician order readily available can provide an additional layer of assurance. 

Bracelet Assist Tool (Optional)

The Wisconsin DNR bracelet’s lobster clasp can be difficult for some older adults.  A $4.95 Bracelet Assist tool, available when ordering, makes clasping easier.

QR Wallet Card for Family Members (Optional)

You can also order a $14.95 aluminum wallet card engraved with your name and a QR code.  When scanned with a smartphone, it will display a copy of your Do Not Resuscitate order.  Some people give one to a spouse or close family member as a backup.
