About StickyJ Medical ID DNR Bracelets
StickyJ Medical ID provides specialized identifier bracelets designed to help Emergency Medical Services (EMS) personnel in Alaska locate your Do Not Resuscitate (DNR) order quickly. The bracelets include instructions for EMS to check your purse or wallet for your DNR document.
QR Code Option for Quick DNR Access
For a modest additional fee, StickyJ Medical ID offers an optional QR Code feature with your medical ID bracelet. If you select this option, you can send us a copy of your DNR order, which we will securely digitize and upload to our protected online database. The QR Code, engraved on the back of your bracelet, allows paramedics and emergency responders to use a smartphone to scan and immediately view an image of your DNR order. This can be extremely useful when time is of the essence.
To add the QR Code option, you must either scan-and-upload OR mail a copy of your Do Not Resuscitate order to StickyJ Medical ID. Once received, we will securely store your document online, making it accessible to EMS personnel whenever necessary. (Note: if you are planning to scan and upload your DNR, we recommend using Adobe Scan. It's a free app available for both Apple and Android phones).
Features of the Alaska DNR Bracelet
The Alaska Do Not Resuscitate ID bracelet by StickyJ Medical ID offers peace of mind by ensuring EMS personnel are aware of your DNR status and know exactly where to locate your DNR documentation. The front of the metal bracelet features the internationally recognized emergency medical symbol and information about the location of your DNR order. The back of the bracelet provides space for several lines of engraving, where you can include your name and an emergency contact phone number.
When ordering your bracelet, please specify whether your DNR order is kept in your purse or your wallet by selecting “DNR in Purse” or “DNR in Wallet.” For questions, you may contact StickyJ Medical ID at 727-823-9500.
If you prefer to order by mail, you can download a DNR bracelet order form by clicking on

Please note: The bracelet alone is not legally binding on Emergency Medical personnel to withhold resuscitation. If you suffer a cardiac event, you must have a Do Not Resuscitate Order with you that can be found by Emergency Medical Personnel. Do Not Resuscitate Orders can only be issued by a medical professional. Some agencies approved by your state’s Department of Health may offer a Do Not Resuscitate bracelet option as well. Please seek the advice of your physician to learn about your options.
