StickyJ Medical ID offers an optional QR Code engraving on the back of your medical ID bracelet for a one-time fee of $9.95. This QR Code links directly to a secure digital copy of your POLST order, stored in our protected online database. In an emergency, paramedics and first responders can scan the code with their smartphone to instantly access your POLST—providing critical information when every second counts.
To include the QR Code with your bracelet:

When prompted, select “Yes, add QR Code.” (Note: this option is selected by default.)

If you prefer not to include a QR Code, simply choose “No” when prompted.
If you selected 'Yes', you can submit your POLST order in one of two ways:

Upload a scanned copy during the ordering process (Note: if you are planning to scan and upload your POLST form, we recommend using Adobe Scan. It's a free app available for both Apple and Android phones). -OR-
