About StickyJ Medical ID DNR and MI-POST Bracelets
The statutes of Michigan require that your DNR bracelet must contain the message Michigan DO NOT RESUSCITATE ORDER on the front of the bracelet.
- On the back it must have the declarant's Name and Address on the first three lines, followed by the name and phone number of the attending physician, if any, on lines 4 and 5.
There are two different types of Do Not Resuscitate Order forms for Michigan -- one for patients who execute the order after consultation with their doctor, and one for others whose religious affiliation members do not seek medical guidance from doctors. We must be provided with a copy of the signed DNR form that was properly executed for the declarant before your order can be released.

The bracelets include instructions for EMS to check your purse or wallet for your medical order.
QR Code Option for Quick DNR or MI-POST Access
For a modest additional fee, StickyJ Medical ID offers an optional QR Code feature with your medical ID bracelet. If you select this option, you can send us a copy of your DNR or MI-POST order, which we will securely digitize and upload to our protected online database. The QR Code, engraved on the back of your bracelet, allows paramedics and emergency responders to use a smartphone to scan and immediately view an image of your DNR or MI-POST order. This can be extremely useful when time is of the essence.
To add the QR Code option, you must either scan-and-upload OR mail a copy of your Do Not Resuscitate or MI-POST order to StickyJ Medical ID. Once received, we will securely store your document online, making it accessible to EMS personnel whenever necessary. (Note: if you are planning to scan and upload your DNR, we recommend using Adobe Scan. It's a free app available for both Apple and Android phones).
Features of the Michigan Bracelets
The Michigan Do Not Resuscitate and MI-POST ID bracelets by StickyJ Medical ID offer peace of mind by ensuring EMS personnel are aware of the existence of your medical orders and know exactly where to locate them. The front of the metal bracelet features the internationally recognized emergency medical symbol and for MI-POST it includes information about the location of your portable medical order. The back of the bracelet provides space for several lines of engraving, where you can include your name and an emergency contact phone number.
When ordering your bracelet, please select the DNR bracelet (CR5080C) if your physician order is simply a Do Not Resuscitate form. Select the Do Not Resuscitate MI-POST bracelet (CR5080DNMIC) if your physician order is an MI-POST with 'No CPR' checked, select the EMS MI-POST bracelet (CR5080MIC) if your physician order is an MI-POST with 'No CPR' left unchecked (indicating you want to be resuscitated following a cardiac event). Important Legal Notice
Please be aware that the bracelet itself is not legally sufficient for EMS personnel to withhold resuscitation efforts. In the event of a cardiac emergency, you must have a copy of your actual Do Not Resuscitate Order with you, and it must be accessible to emergency responders. Only a medical professional can issue a Do Not Resuscitate Order. Additionally, some state-approved agencies may offer their own DNR bracelet options.
