In Hawaii it's required that the patient has a copy of their POLST order with them at all times when out in the community, but to insure that EMS complies with your POLST order, it's necessary to direct them to the location of your completed POLST form. When ordering, be sure to specify whether you want the bracelet to reference wallet or purse, and be sure to always carry your POLST document with you wherever you go.
