StickyJ Medical ID Jewelry is the authorized provider of Kansas Do Not Resuscitate identification bracelets. We will engrave the patient’s Name on the first line of the back of the bracelet, a unique 6-digit identifier on line 2, and we'll engrave the toll-free phone number of our DNR verification service on line 3 of the back of the DNR ID (we provide this information).
- A copy of your Kansas DNR order form (signed by your physician) must be provided to StickyJ Medical ID prior to releasing your order.
- You can either scan the document and upload it at the time you place your order, email it to us to CustomerService@stickyj.com, or fax it to 727-499-0989.
StickyJ Medical ID also provides Kansas TPOPP bracelets for those with an Iowa Transportable Physician Order for Patient Preferences. For a TPOPP bracelet it is important to specify if the physician order is kept in your purse or wallet, since TPOPP orders must be carried with you at all times and must be accessible to EMS personnel in the event of an emergency. We offer separate products for DNR and TPOPP.

QR Code Option for Quick DNR Access
For a modest additional fee, StickyJ Medical ID offers an optional QR Code feature with your DNR or TPOPP medical ID bracelet. If you select this option, when you send us a copy of your physician order, we will securely digitize and upload to our protected online database. The QR Code, engraved on the back of your bracelet, allows paramedics and emergency responders to use a smartphone to scan and immediately view an image of your DNR or TPOPP order. While the bracelet itself communicates your wishes, having the actual DNR or TPOPP order readily available can provide an additional layer of assurance. A scan of the QR code allows EMS personnel to quickly view your physician order on their smartphone, further safeguarding your preferences in emergency situations.
To add the QR Code option, you must either scan-and-upload OR mail a copy of your Do Not Resuscitate or TPOPP order to StickyJ Medical ID. Once received, we will securely store your document online, making it accessible to EMS personnel whenever necessary. (Note: if you are planning to scan and upload your DNR, we recommend using Adobe Scan. It's a free app available for both Apple and Android phones).
