StickyJ Medical ID Jewelry is authorized by the Maine Department of Health as a vendor of Do Not Resuscitate identification bracelets and pendants. We will engrave the patient’s Name on the back of the bracelet, plus a unique 6-digit identifier and the toll-free phone number of our DNR verification service (provided by us). A copy of your Maine DNR order form (signed by your physician) must be provided to StickyJ Medical ID prior to releasing your order.
