We will engrave our 24-hour toll-free verification number (provided as a service by StickyJ Medical ID for no additional cost) and the patient-s ID number (assigned by StickyJ) on the top two lines. You must provide the patient's full name on the first line of the Personalization Options. Then, you may provide additional information, such as an emergency contact number, on subsequent lines. After purchasing this product, note the order number and e-mail a copy of a valid DNR or POLST form for the patient to customerservice@stickyj.com or fax it to (727) 490-5446 (please provide the order number in the e-mail or on a separate piece of paper if faxing).
