StickyJ Medical ID Jewelry is a provider of Iowa Do Not Resuscitate (DNR) bracelets that comply with state regulations. The identifier bracelet is the key way health care providers and/or EMS personnel can quickly recognize that you have an Out-of-Hospital Do-Not-Resuscitate order. It's recommended that the patient's first and last name be engraved on the back of the bracelet in the top line available. A copy of your completed Iowa Out of Hospital DNR Order must be provided to StickyJ Medical ID prior to releasing your order.
StickyJ Medical ID is also a provider of Iowa IPOST bracelets. For IPOST, it is required that you have a copy of your IPOST order with you at all times. When ordering please select either Purse or Wallet to indicate where EMS personnel can find a copy of your IPOST.
When ordering either a DNR or IPOST bracelet, you must send us a copy of your physician order before we can release the bracelet to you.

QR Code Option for Quick DNR Access
For a modest additional fee, StickyJ Medical ID offers an optional QR Code feature with your medical ID bracelet. If you select this option, when you send us a copy of your DNR or IPOST order, we will securely digitize and upload it to our protected online database. The QR Code, engraved on the back of your bracelet, allows paramedics and emergency responders to use a smartphone to scan and immediately view an image of your DNR or IPOST order. While the bracelet itself communicates your wishes, having the actual DNR or IPOST order readily available to emergency medical personnel can provide an additional layer of assurance. A scan of the QR code allows EMS personnel to quickly verify your medical directives, further safeguarding your preferences in emergency situations.
To add the QR Code option, you must either scan-and-upload OR mail a copy of your Do Not Resuscitate order to StickyJ Medical ID. Once received, we will securely store your document online, making it accessible to EMS personnel whenever necessary. (Note: if you are planning to scan and upload your DNR, we recommend using Adobe Scan. It's a free app available for both Apple and Android phones).
